PRIVACY POLICY

Sweetwater Hydration (Doctrlos, PC)

Effective date: October 27, 2026

1. Who we are and what this Policy covers

This Privacy Policy explains how we collect, use, and share personal information when you visit www.sweetwateriv.com
(the “Site”), book or buy services through it, or communicate with us by email, phone, or text. “We,” “us,” and “Sweetwater” mean Doctrlos, PC, a California professional medical corporation doing business as Sweetwater Hydration, which provides all medical services, and Sweetwater Hydration Management, LLC, the management company that operates the Site and provides administrative support to the practice under a HIPAA business associate agreement.

2. Your health information is covered by our Notice of Privacy Practices

Information you give us in order to receive care, including your health history, intake and consent forms, prescriber evaluations, and treatment records, is protected health information under the federal Health Insurance Portability and Accountability Act (HIPAA) and California's Confidentiality of Medical Information Act. How we use, share, and protect that information, and your rights regarding it, are described in our Notice of Privacy Practices, which you receive before your first treatment and which is available on the Site. If anything in this Privacy Policy conflicts with the Notice of Privacy Practices regarding health information, the Notice controls.

3. Information we collect

Information you give us may include your name, email address, phone number, service address, date of birth (to confirm you are 18 or older), appointment and membership details, payment card information (which goes directly to our payment processor),
the contents of messages you send us, and account login details if you create an account. Health information you provide through our intake forms is handled as described in section 2.

Information collected automatically: when you use the Site, we and our service providers automatically collect your IP address, the approximate location derived from it, your browser and device type, the pages you view, the dates and times of your visits,
and the website that referred you, using cookies and similar technologies (see section 5).

Information from others: our payment processor confirms your payments to us, our booking and scheduling tools send us your appointment details, and if you interact with us on social media we may receive information from that platform according to its settings and policies.

4. How we use information

We use personal information to schedule, confirm, and provide your services; to communicate with you about your appointments, account, and membership, including the renewal notices California law requires; to process payments and prevent fraud; to respond to your questions; to send you news and offers by email if you have signed up for them (you can unsubscribe at any time); to operate, secure, and improve the Site; and to comply with our legal obligations. We do not use your health information for marketing without your written authorization.

5. Cookies and analytics

The Site is built on the Wix platform, which uses cookies that are necessary for the Site to work, such as cookies that keep you logged in and support bookings and checkout. We may also use analytics tools, such as Wix Analytics and Google Analytics, that place cookies to help us understand how visitors use the Site. You can control cookies through your browser settings; if you block cookies, some parts of the Site, including booking, may not work. The Site does not respond to browser “Do Not Track” signals. Analytics and advertising providers may collect information about your online activities over time and across other websites when you use the Site.

6. How we share information

We share personal information only as follows: with service providers that help us operate, such as Wix (website hosting, bookings, member accounts, and forms), our payment processor, Google Workspace (email and calendar), and any text-messaging provider we use, each of which may use the information only to provide services to us and, where health information is involved, has signed a business associate agreement with us; between Doctrlos, PC and Sweetwater Hydration Management, LLC as needed to run the practice; when required by law, such as in response to a subpoena, court order, or request from a government agency, or to protect the rights, property, or safety of our patients, our staff, or others; and in connection with a sale, merger,
or reorganization of the practice or the management company, in which case the recipient will be bound by this Policy.

We do not sell your personal information, and we do not share it with third parties for their own marketing purposes. Mobile phone numbers and text-messaging opt-in information are never shared with third parties or affiliates for marketing
or promotional purposes.

7. Text messages

If you give us your mobile number and opt in, we may send you appointment reminders and service updates by text message. Consent is not a condition of purchase. Message and data rates may apply, and message frequency varies. Reply STOP to any message to opt out, or HELP for help.

8. Your choices and rights

You can unsubscribe from marketing emails using the link in any email, opt out of text messages by replying STOP, and manage cookies through your browser. You may ask us to access, correct, or delete the personal information we hold about you by emailing info@sweetwateriv.com; we will verify your identity and respond within 45 days. Requests regarding your medical records are handled under the Notice of Privacy Practices and California medical-records law.

9. How we protect and keep information

We use reasonable administrative, technical, and physical safeguards to protect personal information, including HIPAA-compliant settings for the systems that hold health information. No method of transmission or storage is completely secure,
so please do not send health information through the Site's general contact form or through social media; use the secure intake forms we provide, or call us. We keep personal information for as long as needed for the purposes described in this Policy and
as required by law; medical records are kept for the periods California law requires.

The Site and our services are intended for adults 18 and older. We do not knowingly collect personal information from anyone under 18. If you believe a minor has given us personal information, email info@sweetwateriv.com and we will delete it.
